29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeds one fourth inch: a) The employer, West Side Fabrication, LLC, does not ensure that the distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top never exceeds one fourth inch. On or about March 6, 2018 and at times prior, employees of West Side Fabrication, LLC were exposed to this hazard while operating the Ryobi bench grinder which was missing a tongue guard. West Side Fabrication, LLC was previously cited for a violation of this Occupational Safety and Health standard which was contained in OSHA Inspection Number 1261379, Citation Number 3, Item Number 1b, and was affirmed as a final order on February 7, 2018, with respect to the same bench grinder at the same workplace located at 2120 Naegele Rd., Colorado Springs, CO. West Side Fabrication LLC was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 1134089, citation number 2, item number 1b, and was affirmed as a final order on May 16, 2016, with respect to the same bench grinder at the same workplace located at 2120 Naegele Rd., Colorado Springs, CO.
