29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: a) 700 New Castle Avenue, Wilmington, DE - On or about March 6, 2018 the employer failed to provide a safe means to perform hoisting operations while employees were working on top of a stack of chemicals, without any means of fall protection. The employees were exposed to a fall hazard of approximately 8 to 10 ft.
