29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: 730 Worcester St, Springfield, MA On March 20, 2018, two employees performing maintenance activities were exposed to pinch point hazards of an unguarded hold back piston of the B1 pallet dispenser. While troubleshooting the B-1 pallet dispenser, an electrician employee reached into an area of a pinch point hazard where alternative protective measures were not in place. Solutia was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.147(c)(4)(i), which was contained in OSHA inspection number 1163019, citation number 1, item number 1a and was affirmed as a final order on December 14, 2016, with respect to a workplace located at 730 Worcester St, Springfield, MA.
