Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm, such as crushing and/or struck-by injuries to employees removing banding and plastic from palletized glass sheets. (a) On March 20, 2018, the employer did not ensure that an employee cutting the banding on a pallet of glass sheets in the FBT 2 Staging Area, used an angle gage and T-bar/safety bar to prevent the glass from falling forward and striking the employee. (b) On March 22, 2018, the employer did not ensure that employees cutting the banding on pallets of glass sheets by Post AK 59 in the Tempering Department, used angle gages to prevent the glass from falling forward and striking the employees. (c) On March 22, 2018, the employer did not ensure that an employee removing plastic from a pallet of glass sheets by Post AK 59 in the Tempering Department, used a T-bar/safety bar to prevent the glass from falling forward and striking the employee. A feasible method of abatement may include, but not be limited to: (a) Rewriting the Raw Glass Unboxing (FYUS/PE-WIT-134) procedure, using terminology that is easily understood by employees, such as using terms T-bar or safety bar, for fixing bracket or rack arm, and properly training employees on the procedures use and enforcing the procedures use. (b) Develop an inspection program for the raw glass brackets and the T-bars/safety bars, to include inspecting the T-bar/safety bar holder for damage.
