29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): a) D-10 Cooler; On or about March 15, 2018, and at times prior, the employer exposed employees to struck by hazards in that powered industrial truck operator training and evaluations had not been provided to employees operating the Crown pallet jack #126 (PE 4000 Series Type E, SN 6A279815) to manipulate loads of groceries.
