29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed: The employer is failing to protect employees from amputation hazards when working near sprocket wheels and chains. This was most recently observed at 13027 Renfro Circle, Omaha, Nebraska on March 13, 2018 when a sprocket wheel was observed that had not been guarded or enclosed. On March 6, 2018, an employee sustained an amputation injury when they came into contact with the unguarded chain and sprocket.
