29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: a) East Building: Ilapak Vegatronic 500 (S/N #0680700045): The opening on the guard for the heat sealer (pressure bar) and the sealant element had an opening of approximately 15.5 inches in width and 8.5 inches in length, allowing for employees to potentially reach into the danger zone during the operation cycle, exposing employees to a potential caught-in/between hazard, observed on or about March 15, 2018. b) Specialty Area: Two Jasa packaging machines (unknown Model): The opening on the guard for the heat sealer (pressure bar) and the sealant element had an opening of approximately 18 3/4 inches in length and about 31 inches in width, allowing for employees to potentially reach into the danger zone during the operation cycle, exposing employees to caught-in/between hazard, observed on or about March 15, 2018.
