29 CFR 1910.178(m)(6): A safe distance was not maintained from the edge of ramps or platforms while on any elevated dock, or platform or freight car. a) On or about March 4, 2018, at warehouse Dock #6, during the operation of a Heli forklift while lifting and pushing a steel plate, the forklift was pulled over the edge of the dock falling to the ground level below.
