29 CFR 1910.119(f)(4): The employer did not develop and implement safe work practices to provide for the control of hazards during operations such as lockout/tagout; confined space entry; opening process equipment or piping; and control over entrance into a facility by maintenance, contractor, laboratory, or other support personnel: On or around March 6, 2018 in the OMCC area of the Galveston Bay Refinery, the employer did not implement safe work practices to provide for the control over entry into the facility by maintenance personnel, when: (a) I&E technicians entered into OMCC process area without being signed into the area. (b) I&E technicians entered Load Center 46 and performed work on equipment without approved/endorsed Work Permit(s).
