29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: a. On or about 1:40 pm on 03/02/2018 at 8490 Sheldon Rd. Tampa, Florida, an employee suffered a work related finger amputation and finger injuries and was hospitalized for surgery on the same day of the injury. The employer was aware of the amputation on the same day of the incident. The employer notified OSHA on 03/06/2018 at 2:40 pm of the amputation and in-patient hospitalization.
