The employer was not performing inspections with a regularity necessary to identify the hazards of employees fall arrest lanyards or cross arm straps not protected from sheering/abrasion free/swing falls against the edge of platforms, fall arrest systems allowed employee(s) to strike lower levels, horizontal lifelines were not designed, installed, and used as a compliant fall arrest system, lack of conventional fall protection systems utilization after assessing stages and during shepherds hook transfer associated operations, incompatible connection surfaces allowing accidental disengagement hazards of shepherds hook during end edge stage/platform work, lack of rescue procedures for every location where an active fall protection system is used to control fall hazards and lack of lanyard(s) used connected to horizontal lifeline being capable of locking in both directions.
