29 CFR 1960.8(a): The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm in that an employee was struck by a 1700-pound upper aft escape hatch that was not secured to prevent it from closing after removal of the linkage support bolts. a) Norfolk Naval Shipyard Portsmouth, SSN 711: On or about 02/27/2018, an employee was accessing the aft escape truck when a 1700-pound hatch unintentionally closed striking the employee. The upper aft escape hatch was not secured before the foundation bolts that supported the spring canister linkage were removed. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
