Instance a) On or about February 27, 2018 at 490 Road 9, Schuyler, Nebraska 68661, at the Cargill Meat Solutions facility, the employer did not document that it complied with recognized and generally accepted good engineering practice (RAGAGEP), such as ASHRAE 15 (2013), Section 8.13(f), when it did not ensure that several wall penetrations for piping and conduit were sealed to prevent ammonia from communicating from the ammonia engine/compressor room to the adjacent boiler room, exposing employees to the hazards of inhalation of toxic ammonia and ammonia burns.
