29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: Building 1B: At this establishment, on or about February 15, 2018, lockout or tagout devices had not been affixed to the electrical energy source of the E-8 exhaust fan by authorized employees prior to the employees working with the units barometric dampeners. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date, and method of abatement, and states employees and their representatives were informed of this abatement.
