29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Production area, Room 1: The 3761 Cartoner machine #32 (Ceco 42 HN/SN 3023) was inadequately guarded in that workers were exposed to contact with rotating parts and a worker sustained a severe arm injury while attempting to clean beneath the machine, on or about February 19, 2018. b) Production area, Room 1: The front portion of machine R6 was not properly guarded exposing workers to contact with rotating parts, as observed on or about February 23, 2018. c) Production area, Room 4: The moving flight chain and pushers located at the rear of machine R1 were not properly guarded exposing workers to contact between the moving pushers and machine frame, as observed on or about February 23, 2018. North American Packaging dba Econo-Pak was previously cited for a violation of this Occupational Safety and Health standard which was contained in OSHA inspection number 1165278, citation number 1, item number 1a and was affirmed as a final order on March 1, 2017, with respect to a work place located at 535 Route 6 & 209, Milford, PA.
