Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1910.23(b)(9): Ladders were not inspected before the initial use in each work shift, or more frequently as necessary, to identify any visible defects that could cause injury: a) Ohio Pickling and Processing located at 1149 Campbell Street in Toledo, Ohio; On or about February 18, 2018 the employer did not ensure employees inspected ladders prior to initial use. Employees were exposed to fall hazards in that the rubber feet of the Werner 25 foot extension ladder were worn down to the steel thus reducing the grip of the ladder on the smooth epoxy floor.
