29 CFR 1910.147(a)(2)(ii)(A): Alternative measures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by the exception of 1910.147(a)(2)(ii) for routine, repetitive, and integral processes. At the worksite located at 5750 EAST Front Street, Kansas City, Missouri. Employees in the production area on line 21, were exposed to serious amputation, crushing and struck-by injuries, in that employees were required as a repetitive process to clear jams to place part of their body into an area on the machine known to the company as Uni-Loy 2011, at the point of operation. Alternative measures were not documented at the time of inspection.
