29 CFR 1910.212(a)(3)(ii): Point of operation of machinery were not guarded or guards were not designed and constructed to prevent employee(s) from having any part of their body in the danger zone during operating cycles: (a)Twin City Fan Companies, LTD at 201 Badger Street North, Elkton, SD 57025: On or about and at times prior to January 10, 2018, the employer did not ensure that the point of operation regarding the Amada Astro 100 NT(Robotic) brake press was effectively guarded. On January 10, 2018, an employee actuated the foot treadle causing the brake press to cycle while attempting to retrieve finished parts between the ram and lower press brake die opening, causing crushing injuries to four fingers on the right hand. This condition exposed employees to crushing and amputation hazards. Twin City Fan Companies, LTD was previously cited for violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 1068660, citation number 1, item number 1 and was affirmed as a final order on March 17, 2016, with respect to a workplace located at 101 Sunrise Ridge Road South, Brookings, SD 57006.
