29 CFR 1926.21(b)(2): The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury. On or about January 22, 2018 at the W249 Deer Drive in Pulaski, Wisconsin jobsite, employees were not trained to recognize and avoid the area around delivery trucks while being unloaded exposing employees to struck by and crushing injuries.
