On or about January 22, 2018 at Hwy 30-A (Cypress Dunes) Lot 41 in Santa Rosa Beach, FL, the employer did not ensure that employees successfully completed training to recognize hazards associated with dismantling the scaffold, exposing employees to struck-by, crushing, and fall hazards.
