OSH ACT of 1970 Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and fall hazards: a. On or about January 21, 2018, employees were exposed to struck-by and drowning hazards due to corroded components of the pneumatic pressure system, such as but not limited to, the isolation valve handle on a riser. The mechanical integrity of the pneumatic pressure system and its components were not maintained in good working order.
