29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a)Egger Steel Company at 909 South 7th Avenue, Sioux Falls, SD 57104: On or about and at times prior to December 8, 2017, the employer did not ensure employees were protected from crushing injuries, in that employees were not protected from the point of operation associated with a die change-out on the Cincinnati 400Hx12 Press Brake. An employee suffered a fingertip amputation during a die change-out after the employee contacted the foot treadle initiating the press to cycle while the employees finger was between the bed and the lower die. This condition exposed an employee to amputation hazards associated with die change-out.
