29 CFR 1904.29(a): A log of all Work-Related Injuries and Illnesses (OSHA Form 300), and/or the Summary of Work-Related Injuries and Illnesses, (OSHA Form 300A) and/or the Injury and Illness Incident Report (OSHA Form 301) or equivalent forms were not kept by the establishment: (a) Calendar year 2017: The employer did not maintain an OSHA log of work related injuries and illnesses (OSHA 300) and an annual summary (300A). The employer had a recordable work place injury on 11/17/2017 which resulted in two (2) days of work related restriction. (b) Calendar year 2017: The employer did not maintain an OSHA log of work related injuries and illnesses (OSHA 300) and an annual summary (300A). The employer had a recordable work place injury on 12/19/2017 which resulted in seven (7) days of work related restriction. Note: Abatement certification IS required for this item.
