Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1910.178(m)(2): No person shall be allowed to stand or pass under the elevated portion of any truck, whether loaded or empty. (a) On or about December 10, 2017 for an employee who was injured after being struck by the pipe grappler attachment on a front end loader while he was acting as a signal person for the loader operator at the Xtreme 16 CPEUSC Keith 2-16-21-158N-101 W well site near Williston, North Dakota. Abatement note: Among other feasible and acceptable means of abatement, the employer can ensure employees follow the manufacturers guidance on page 1-3-2 of the operators manual which advises to keep bystanders away from boom attachments and to ensure that the signal person be kept in view by the operator at all times. Additionally, the loader operators to ensure they are competent in relevant skills such as those found in the IADC Front-End Wheel Loader Operator Competency Criteria. Specifically, item 4a which advises that operators be able to demonstrate the ability to control an area and position personnel where lifting operations are being carried out.
