29 CFR 1910.146(c)(6): When there were changes in the use or configuration of a non-permit confined space that could increase the hazards to entrants, the employer did not reevaluate that space and, when necessary, reclassify it as a permit-required confined space On December 23, 2017, the employer did not ensure the clarifier tank that employees were required to perform sand blasting in was re-evaluated and reclassified as a permit-required confined space when garnet sand (respiratory irritant) and a dust collector (asphyxiation) were introduced to the space.
