29 CFR 1926.501(b)(4)(i): The employer did not ensure that each employee on walking/working surfaces was protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. This violation occurred on December 23, 2017, employee(s) were not protected from falling through skylights and exposed to a fall of approximately 40 to the concrete below.
