29 CFR 1910.132(f)(1): The employer did not provide training to each employee who is required by this section to use personal protective equipment: a) Sundial Lodge- Quick Jump Area, 1001 Lafayette Drive, Farmington, PA 15437: On or about December 21, 2017, employees did not receive training on the proper selection and use of personal fall arrest equipment used while on the elevated platform. Employees were subjected to fall and ejection hazards from the platform.
