Text For Citation: 01 Item/Group: 001 Hazard:
Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to caught-in hazards: (a) On or about December 20th, 2017 for an employee who suffered an amputation of one finger and injuring another finger while attempting to hook up a chain on the back of a Selco vertical down stroke baler, Model # V5-HD, Serial # 05893629, while the machine was in operation, located at 3000 Seventh Avenue in Fargo, North Dakota. Abatement Note: Among other methods, one feasible and acceptable abatement method to correct the hazard would be for the employer to follow manufacturer instructions along with the American National Standard Institute (ANSI) standards to include, but not limited to ensuring that all personnel are clear of the point of operation before actuating any cycle or controls.
