29 CFR 1910.106(b) (6): In locations where flammable vapors might present, precautions were not taken to prevent ignition by eliminating or controlling sources of ignition: (a) Colter Energy Services, Inc., at the Stromberger/22-E Pad, SENE 226N 67W, Windsor, CO 80550: On or before December 26, 2017, the employer did not ensure that employees were protected from injury, in that on December 22, 2017 an employee sustained severe burn injuries due to a fire that occurred near high-pressure separators and flow-back storage tanks that contained water, oil, gas, and sand during flow-back operations. A non-spark proof Multiquip Power WhisperWatt Super-Silent Generator, Model# DCA25SSIU4F and Serial# 7152946, was located approximately 10 ft. from the separators and storage tanks. This condition exposed employees to fire and explosion hazards.
