29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours. On 12 December 2017 at approximately 4:00 PM CDT, an employee sustained an amputation of the left thumb tip while cutting small pieces of wood with a radial arm saw for floor truss webbing. The employee subsequently remained in the hospital overnight as well. The incident was not reported to the local area Occupational Safety & Health Administration office until 14 December 2017 at approximately 3:22 PM CDT.
