29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Proof Tester 43, OFS Fitel, 50 Hall Road, Sturbridge, Massachusetts; On and before December 11, 2017 the employer did not utilize the energy control procedure while threading the proof tester, exposing employees to crushing and amputation injuries during the setup operation.
