29 CFR 1910.23(b)(10): The employer did not ensure that any ladder with structural or other defects was immediately tagged "Dangerous: Do Not Use" or with similar language in accordance with �1910.145 and removed from service until repaired in accordance with �1910.22(d), or replaced: This violation was most recently observed on or about December 6, 2017, and at times prior thereto in the Benzoyl Peroxide (BPO) Unit at United Initiators located in Helena, Arkansas, when the employer failed to correct structural equipment deficiencies; corroded and unsecured supports for the fixed metal ladder used to access the platform above centrifuges.
