29 CFR 1926.754(e)(2)(ii): Roof and floor holes and openings shall be decked over. (a) 4 Seasons Roofing & Sheet Metal, at 5555 N Franklin Street, Denver, CO: On and preceding 11/29/17, the exposing employer did not ensure that roof holes and openings were decked over. Employees removed and replaced damaged metal decking on the roof of a structure. An employee stepped into a roof opening and fell approximately 23 feet to the ground. This condition exposed employees to a fall hazard.
