29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: Location: Intersection of Turnpike Street & Maple Street, Stoughton, MA On of about November 30, 2017 employees were exposed to cave-in hazards, where the employer did not train the employee to recognize or avoid the hazards of working in a 8 foot deep trench without cave-in protection.
