These safe work practices shall apply to employees and contractor employees. On or about November 22, 2017, at Invista Plant in La Porte, Texas; the employer did not implement the Safe Work Permits procedure to issue and/or retain the work permit, including but not limited to the Blanking Lists, Job Safety Analysis (JSA) for the maintenance activities on Step II C Reactor.
