29 CFR 1910.119(f)(4): The employer did not implement safe work practices to provide for the control of hazards during operations. These safe work practices shall apply to employees and contractor employees. On or about November 22, 2017, at Invista Plant in La Porte, Texas; the employer did not implement the Safe Work Permits procedure to issue and/or retain the work permit, including but not limited to the Blanking Lists, Job Safety Analysis (JSA) for the maintenance activities on Step II C Reactor.
