Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Department 15: On, or about, December 27, 2017, adequate safeguarding had not been provided on the following machines: A. Machine #18, Roll-threader: An opening was left on the left side of the chute that allowed access to the rotating tooling. B. Machine #33, G.M. Diehl horizontal lathe: A chuck guard was not provided and an emergency stop device was not within reach of the operator during the operation of the machine. C. Machine #27, Horizontal piercing machine: The control buttons were not ringed or otherwise protected and the point of operation was within reach of the operators station. D. Blue Auto Stamper: The plexiglass guard that prevented access to the area between the upper die and ram appeared to be broken off, allowing access to the pinch point. E. Machine #29, Walker-Turner drill press with two tapping heads: A section of the guard that had been provided to prevent access to the rotating tapping heads was missing from the machine. F. Machine #25, small Denison hydraulic press: The two control buttons were not ringed or protected against accidental operation, the motor start control was not protected against accidental contact and the press was not provided with a red emergency stop button. G. Machine #20, small Denison hydraulic press: The motor start control was not protected against accidental operation and the press was not provided with a red emergency stop button. H. Machine #1, Denison hydraulic press: This press was provided with two hand controls, however, as the ram descended, a limit switch was activated that took over the machine control so that the operator could release the buttons and grab the next part. The operator could release the hand controls before the tooling had closed, exposing the operator to a pinch point.
