29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): On or about November 16, 2017 employees operating the Cincinnati press brake were exposed to amputation hazards because the opening between the upper and lower die was not guarded to prevent employees from placing their hands in to the point of operation during operation.
