29 CFR 1910.119(d)(3)(i)(B): Process safety information pertaining to the equipment in the process did not include the piping and instrument diagrams (P&IDs) On or about February 5, 2018, and at times thereto, Piping and instrument diagrams such as, but not limited to the following, were inaccurate or incomplete: a) According to Drawing MR3.0, Compressor 1 Thermal Syphon which is protected by Pressure Relief Valve (PRV) CMP1 104, reflects a different set pressure for the PRV than what was observed in the field. b) According to Drawing MR3.0, Compressor 2 Thermal Syphon which is protected by Pressure Relief Valve (PRV) CMP2 104, reflects a different set pressure for the PRV than what was observed in the field. c) According to Drawing MR3.0, Compressor 3 Thermal Syphon which is protected by Pressure Relief Valve (PRV) CMP3 104, reflects a different set pressure for the PRV than what was observed in the field. d) According to Drawing MR3.0, Compressor 4 Thermal Syphon which is protected by Pressure Relief Valve (PRV) CMP4 104, reflects a different set pressure for the PRV than what was observed in the field. e) According to Drawing MR3.0, Compressor 5 Thermal Syphon which is protected by Pressure Relief Valve (PRV) CMP5 104, reflects a different set pressure for the PRV than what was observed in the field. f) According to Drawing MR3.0, Compressor 6 Thermal Syphon which is protected by Pressure Relief Valve (PRV) CMP6 104, reflects a different set pressure for the PRV than what was observed in the field. g) During field observations, Compressor 1 was observed to be protected by Pressure Relief Valve (PRV) CMP1 500, but Drawing MR3.0 does not reflect the PRV, tag number, or the set pressure for the PRV. h) During field observations, Compressor 2 was observed to be protected by Pressure Relief Valve (PRV) CMP2 500, but Drawing MR3.0 does not reflect the PRV, tag number, or the set pressure for the PRV. i) During field observations, Compressor 3 was observed to be protected by Pressure Relief Valve (PRV) CMP3 500, but Drawing MR3.0 does not reflect the PRV, tag number, or the set pressure for the PRV. j) During field observations, Compressor 4 was observed to be protected by Pressure Relief Valve (PRV) CMP4 500, but Drawing MR3.0 does not reflect the PRV, tag number, or the set pressure for the PRV. k) During field observations, Compressor 5 was observed to be protected by Pressure Relief Valve (PRV) CMP5 500, but Drawing MR3.0 does not reflect the PRV, tag number, or the set pressure for the PRV. l) During field observations, Compressor 6 was observed to be protected by Pressure Relief Valve (PRV) CMP6 500, but Drawing MR3.0 does not reflect the PRV, tag number, or the set pressure for the PRV. m) The Heat Exchanger Liquid Subcooler (HE-2) is protected by a Pressure Relief Valve (PRV), but does not have an assigned tag number in the field or on Drawing MR3.0. n) The Low Pressure Receiver -18degF Oil Pot (-18degF LPR-1 Oil Pot) is protected by a Pressure Relief Valve (PRV), but does not have an assigned tag number in the field or on Drawing MR3.0. o) The Heat Exchanger (HE-3) is protected by a Pressure Relief Valve (PRV), but does not have an assigned tag number in the field or on Drawing MR3.0.
