Text For Citation: 01 Item/Group: 001 Hazard:
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: On or about November 14, 2017, at the EM Fenton #9 well, south of Illinois Highway 14, located in a field approximately 3.5 miles east of Crossville, Illinois - Employees were exposed to fire and explosion hazards when ignition sources such as, but not limited to open flames were located approximately 60 feet from a well which was being serviced.
