Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees. a. The open end section of the hose was not properly secured, thereby exposing employees to a struck by hazard. b. On or about November 12, 2017, and at times thereto, the employer did not ensure that employees were protected from unsafe movement and whipping hose hazards while flushing out a clogged discharge hose attached to the high pressure manifold trailer. Employees were striking the pressurized hose with hammers in attempt to break up the blockage, thereby exposing employees to a struck by hazard. c. On or about November 12, 2017, and at times thereto, the employer did not ensure that employees were protected from unsafe movement and whipping hose hazards while flushing out a clogged discharge hose attached to the high pressure manifold trailer. Employees were standing at the discharge end of the hose and sticking a rod up the end of the hose in attempt to break up the blockage, thereby exposing employees to struck by hazard.
