29 CFR 1926.451(e)(9)(i): The employer did not provide safe means of access for each employee erecting or dismantling a scaffold. a) Employees were exposed to 100 ft fall hazards when accessing the tubular welded frame scaffold by climbing on the frames. Employees were altering the scaffold by passing up scaffold planks 15 ft up within the scaffold from 6fl to 7fl level and there was no safe means to access the work areas of the scaffold. --- A scaffold stairway was not provided as required by the scaffold design plan.
