29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: (a) On or about November 4, 2017, an employee working on the FDA Line attempted to remove a jammed loaf of bread from the bread slicer, by reaching under the guard at the exit side of the slicer. This exposed the employee fingers and hand to the point of operation (slicer blades) and amputation injury.
