29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: A. On or about October 20, 2017, Warehouse: The employer did not ensure that each employee working on the racking system in the warehouse with unprotected sides greater than four feet above the lower level, were protected from falls to a lower level. An employee was severely injured when he fell from a rack on the third level of Mod 4, Level C, 17-feet to the concrete floor below.
