29 CFR 1910.1200(e)(1): The Employer did not develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, material safety data sheets, and employee information and training will be met, and which also includes the following: On or about October 15, 2017, employees working on an oil/gas production platform located in Lake Ponchartrain, LA, were engaged in a well servicing pipe cleaning/clearing operation utilizing a proprietary chemical process to remove paraffin wax. Prior to the commencement of the operation, the employer did not analyze and evaluate the known and recognized hazards of the process for the specific application and conditions in which it was used and the potential for fire and explosion.
