29 CFR 1910.28(b)(15): Walking-working surfaces not otherwise addressed. Except as provided elsewhere in this section or by other subparts of 29 CFR part 1910, the employer must ensure each employee on a walking-working surface 4 feet (1.2 m) or more above a lower level is protected from falling by: 1910.28(b)(15)(i), 1910.28(b)(15)(ii), or 1910.28(b)(15)(iii). On or about November 7, 2017, the employer did not ensure that employees who were climbing the metal rack system to remove molds, located in the set up/pouring department, were protected from falling. The employees were exposed to heights of greater than four feet.
