Text For Citation: 01 Item/Group: 001 Hazard:

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment that was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees. In that employees were exposed to struck by and crushing hazards while engaged in overhead crane operations: The employer is failing to protect employees from struck-by and crushing hazards. This was most recently documented on or about November 3, 2017, at the workstation 2-297F Plant II factory located at 3801 S. Oliver St., Wichita, Kansas. Employees engaged in overhead crane operations were exposed to struck by and crushing hazards in the following instances: a. At the workstation 2-297F Plant II factory, the sling hooks were provided without safety latches to securely attach the sling hooks to the load. b. At the workstation 2-297F Plant II factory, the overhead crane operator and the employees on the ground did not utilize a consistent form of communication to include, but not limited to, hand signals and/or radio communication when handling loads. Among other means, one feasible and acceptable method to abate this hazard is to follow what is required by ASME B30.2 - 2011 (Revision of ASME B30.2 - 2005) including but not limited to; Chapter 2-1, Section 2-1.14: Hoisting Equipment, para. 2-1.14.5 Hooks shall meet the manufacturer's recommendations and shall not be overloaded. Latch-equipped hooks shall be used unless the application makes the use of the latch impractical or unnecessary. Chapter 2-3, Operator Training and Operation, Section 2-3.3.3 Responsibilities of Management(Owners/Users), para. 2-3.3.3(b) Provide training to persons who will operate a crane. Chapter 2-3, Signals, Section 2-3.6 Signals, para. 2-3.6.1 (c) Hand Signals shall be posted conspicuously and should be illustrated.
