29 CFR 1910.23(c)(12): The employer did not ensure that ladders and ladder sections were not tied or fastened together to provide additional length unless they were specifically designed for such use: (a) Location - 100 Danton Drive, Methuen, MA: On or about 4/18/2018, the ladder of the ammonia refrigeration system condenser was extended by bolting the base section of a Werner 16 foot extension ladder to it.
