Test voltages and the maximum intervals between tests were not in accordance with Table I-4 and Table I-5. a. Wilbert, Inc. located in Bellevue, Ohio: On or about November 1, 2017, the employer did not ensure electrical protective gloves located in the maintenance department and available for use by employees, were tested every six months.
