29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle. a) Bay 7, Guides, 24 Elmwood Drive, Mountaintop, PA - Employees operated the Edwards Manufacturing Company 120 Ton Jaws Ironworker (Serial No. 17212009) with a bent notcher section guard, creating an opening on the left and right sides of the guard that exposed employees to an amputation hazard between the top and bottom notcher blades, on or about October 28, 2017. In accordance with 29 CFR 1903.19(c), abatement certification and documentation is required for this violation.
