29 CFR 1915.74(a)(2): The employer permitted employees to board or leave a vessel by using a gangway that did not have on each side a railing with a minimum height of approximately 33 inches, measured perpendicularly from rail to walking surface at the stanchion, with a midrail and taught at all times: On October 28, 2017, at the Samoa Tuna Processing Dock: (a) The bottom left railing stanchion on the gangway was only 29 inches high, exposing employees to fall hazards. (b) The railing did not include a midrail, nor was the netting taught, exposing employees to fall hazards. (c) The top right stanchion was not installed and the next stanchion in line was not supported, nor secured, exposing employees to fall hazards.
